As someone who now works off of four e-mail addresses, I find this extension not only useful, but virtually necessary. I'm grateful someone finally decided to design it!
The author of the extension is quick to communicate and promptly fixed a minor bug for mac user's when informed about it.
While john2010's suggestions are intriguing, I would find that setting up the rules for such an extension would take too long, whereas this extension allows me just the right amount of hesitation and choice. Moreover, this extension does exactly what it says it does. That's worthy of five stars.

Thank you! At last I can read .pdf "inline" and can even search through them -- something "Firefox PDF PlugIn for Mac" does not do, and apparently never will. So a happy convert here. My only wish would be able to access this addon in one click instead of two. I'd like the option for this addon to be the default action for .pdf links. For instance, I cannot open any bookmarks that are .pdfs with this addon. Otherwise, long overdue functionality for Firefox/Mac users!! Thank You!

To stop double notifications, disable the extension, restart Thunderbird; enable the extension, restart Thunderbird. Apparently you need to disable/enable the extension once post installation. But it does reduce the growl notifications to only one per message when you do so. I do like the basic functionality of the extension, but it is a shame that the developers don't seem to have enough interest in the extension to fix that basic problem, or to address some of the suggestions to improve it.
