Thank you so much for your feedback and screenshots.
The icons are determined by Firefox and they are generic. In other words Firefox will not show the real icon of the downloaded exe file but a generic icon for the type of the download. As it seems for your OS theme, Firefox places generic white rectangular exe icons next to the file name. I know it seems to be empty but that is icon that your OS theme sets for generic exe icon and those icons are different for each OS. You may want to change OS theme or skins if you do not like current white icons.
